RETAIN Resource: Root Cause Analysis Process Handout and Worksheet

This resource is a video that guides RETAIN programs through the Root Cause Analysis (RCA) Process using the “5 Whys” strategy.  It uses examples to illustrate how RCA might be applied in a RETAIN setting.  The video is accompanied by a handout which include the steps and content aligned with the example depicted in the video, and a worksheet with a second RCA example and guiding questions for your RETAIN team to consider as you conduct an RCA. These resources are designed to support your continuous quality improvement efforts. 

RETAIN Recruitment-to-Enrollment Dashboard and Related Resources

The RETAIN Recruitment-to-Enrollment Dashboard is designed to help you understand the effectiveness of your recruitment and enrollment efforts. The tool uses data obtained by your RETAIN team to produce a dashboard displaying information about the number of potential participants that you connect with at each stage of the recruitment-to-enrollment process—and who ultimately enrolls in the project. The Dashboard tool (link to Dashboard Tool) comes with the following suite of TA resources:
